Systematic Search Strategy

Comprehensive and Reproducible Literature Search Protocol · Also known as: search protocol, systematic search, comprehensive search strategy

A systematic search strategy is a comprehensive, transparent protocol for retrieving all relevant literature addressing a well-defined research question. Developed by the Cochrane Collaboration and formalized in guidelines like PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses), systematic search strategies are essential for conducting unbiased literature reviews, systematic reviews, and meta-analyses. Unlike ad hoc searches (searching Google Scholar or PubMed without a protocol), systematic searches document every step—which databases were searched, what search terms were used, how many results were retrieved, and what inclusion/exclusion criteria were applied—enabling other researchers to reproduce the search and verify that no relevant studies were missed.

When to use it

Systematic search strategies are required for: (1) systematic reviews and meta-analyses (mandatory per PRISMA, Cochrane guidelines), (2) evidence syntheses and rapid reviews (essential for reproducibility), (3) grant proposals requesting funding for literature reviews (funders expect transparent protocols). They are highly recommended for: (4) literature reviews in dissertations, (5) any research that makes broad claims about 'the state of the evidence' on a topic, (6) scoping reviews and narratives reviews where comprehensiveness is important. A fully systematic search is less critical for: (1) narrative essays or opinion pieces, (2) case studies or qualitative research with narrow scope, (3) rapid evidence checks with tight time constraints (though even rapid reviews should document their search process). When in doubt, conduct a systematic search; the effort is modest compared to the credibility gain.

Strengths
  • Transparency: every step is documented, enabling readers to understand exactly what was searched and found, and to replicate the search.
  • Reduces bias: pre-specified inclusion/exclusion criteria and independent screening by multiple reviewers reduce selective article inclusion.
  • Comprehensive: systematic searches across multiple databases and grey literature capture more relevant studies than ad hoc searches.
  • Verifiable: the published search strategy can be scrutinized by peer reviewers and readers, who may identify errors or omissions.
  • Enables updating: a documented search can be re-run at future dates to identify new studies, essential for living reviews and ongoing evidence synthesis.
Limitations
  • Time-intensive: developing and executing a systematic search across multiple databases and screening abstracts and full texts is labor-intensive (typically 100–200 hours for a literature review).
  • Requires expertise: designing an effective search strategy requires familiarity with controlled vocabularies and database syntax; most researchers benefit from consultation with a librarian.
  • Database limitations: not all literature is indexed; grey literature, unpublished data, and non-English publications may be missed despite systematic searching.
  • Language limitations: most researchers search English-language databases; non-English literature is often missed, introducing language bias.
  • Static snapshot: published search strategies capture literature available at the time of search; rapid developments in fast-moving fields may outpace the review.

How many databases should I search?

At minimum, search the primary databases for your field (e.g., MEDLINE for medicine, PsycINFO for psychology). For comprehensive reviews, search 3–5 databases (e.g., MEDLINE + Embase + PsycINFO + CINAHL). Consult a librarian to determine which databases cover your topic best. Searching more databases increases cost and time but improves comprehensiveness.

Should I search Google Scholar?

Google Scholar is not recommended as a primary source for systematic reviews because its coverage is inconsistent and ranking is by relevance, not comprehensiveness. However, you can use Google Scholar to: (1) pilot your search strategy and (2) verify that key known articles appear in results. For the final systematic search, use standard bibliographic databases (MEDLINE, Embase, Scopus) with reproducible search algorithms.

What limits should I apply (publication year, language)?

Apply limits judiciously and justify each. Limits reduce results but may introduce bias. Publication year limits are common ('articles published after 2010') if older evidence is outdated or if you are updating a prior review. Language limits are necessary if you cannot translate, but acknowledge this as a limitation. Study design limits (e.g., 'RCTs only') are appropriate if your question requires experimental evidence. Document all limits in your published search table.

How do I know my search is comprehensive enough?

Use the 'saturation test': after screening titles/abstracts, are new relevant articles still appearing in your results, or have you plateaued? If results are plateauing, your search is likely comprehensive. Also verify sensitivity: have you retrieved known relevant articles? If not, revise your search. For systematic reviews, report search comprehensiveness in your discussion; acknowledge any studies you could not locate or article type you may have missed.
