Living Systematic Review

Living Systematic Review (Continuously Updated Synthesis) · Also known as: LSR, Continually Updated Review, Dynamic Evidence Synthesis

A living systematic review (LSR) is a dynamic, continuously updated evidence synthesis that monitors emerging literature and incorporates new studies as they become available, rather than being a static document published once. Formalized by Elliott et al. (2017) and adopted by the Cochrane Collaboration, living systematic reviews maintain currency in rapidly evolving fields by using prospective searches and regular review cycles (monthly, quarterly, or trigger-based). Rather than waiting 12-18 months for a complete systematic review only to find it outdated by new trials, living reviews enable real-time evidence synthesis—particularly valuable during pandemics, in rapidly advancing fields (oncology, immunology), and for volatile policy questions where new evidence frequently shifts recommendations.

When to use it

Establish a living systematic review when (1) evidence is accumulating rapidly and significant new trials are expected frequently (e.g., during pandemic, for a newly approved drug class, in fast-moving fields like immunotherapy), (2) the evidence base directly informs important clinical or policy decisions, (3) you have resources (team, funding, infrastructure) to maintain continuous updates, (4) the topic is expected to remain relevant for extended periods (not a one-time decision), or (5) guideline developers or policy makers have explicitly requested living evidence to support ongoing decision-making. Living reviews are increasingly common for high-priority areas (COVID-19, vaccines, new oncology treatments) but impractical for low-priority or stable topics.

Strengths & limitations

Strengths
  • Maintains currency: ensures evidence synthesis reflects latest available research, reducing time-lag bias where clinicians unknowingly rely on out-of-date reviews.
  • Responsive to new evidence: when practice-changing trials emerge, living reviews incorporate them promptly, enabling timely guideline updates and clinical practice changes.
  • Reduced redundancy: rather than multiple overlapping systematic reviews being conducted on the same topic as new evidence emerges, a single living review serves all stakeholders.
  • Real-time guidance: particularly valuable during emergencies (pandemics, environmental crises) where decisions cannot wait for traditional review cycles.
  • Evidence of stability: if an living review is updated multiple times and conclusions remain consistent despite new studies, this builds confidence in robustness of evidence.
Limitations
  • Resource-intensive: requires sustained funding, team, and infrastructure over extended periods, which is expensive and may be vulnerable to budget cuts.
  • Methodological complexity: maintaining consistency across many update cycles requires rigorous protocols, careful documentation, and attention to avoid drift in inclusion criteria or analysis methods.
  • Potential fatigue or inconsistency: if leadership or team members change between update cycles, methodology or rigor may drift. Regular audits and protocol adherence checks are essential.
  • Determining update triggers: deciding when to update (on schedule vs when sufficient new studies accumulate) can be challenging. Frequent updates increase currency but increase costs; infrequent updates risk missing important new evidence.

How often should a living systematic review be updated?

Update frequency depends on the topic and available resources. Fast-moving fields (pandemic-related topics, new drug approvals) might update monthly or quarterly. Slower-moving fields might update annually or biannually. You can use event-triggered updates: update whenever a pre-specified number of new eligible studies are identified (e.g., 5-10 studies) rather than on a calendar schedule. Pre-specify the update schedule in your protocol. If you cannot commit to regular updates, do not establish a living review; a traditional review updated every 3-5 years is more appropriate.

How do I ensure consistency across multiple update cycles?

Document all protocols and methods in detail in an initial, published protocol. Create a detailed operational manual for your team covering: how to conduct searches, screen results, extract data, assess quality, and analyze findings. For each update, follow this manual exactly. If modifications to protocol are needed, document and justify them, and communicate changes to stakeholders. Conduct regular audits: randomly check that screening, data extraction, and quality assessment are consistent with protocols. Maintain version control: explicitly document what changed in each update.

What happens if a living review is no longer maintained?

If updates cease, the review should no longer be presented as 'living.' Explicitly state when the last search occurred and when the last update was completed. The review reverts to a static, time-stamped review. Readers can then judge whether it is current enough for their purposes. If you cannot commit to long-term maintenance, avoid establishing a living review format.

Can I transition a traditional systematic review into a living review?

Yes, you can update a published traditional review and convert it to a living format. Publish a protocol for the living review updates, establish a platform to host the continuously updated review, and begin prospective searching and updating on a pre-specified schedule. Clearly communicate the transition to readers.
