IVF Embryo Grading

In Vitro Fertilization Embryo Grading System · Also known as: embryo morphology assessment, embryo viability grading

IVF Embryo Grading is a standardized morphological assessment system for evaluating the quality and viability of embryos in assisted reproductive technology. First formalized by Gardner and colleagues in 1999, it uses microscopic examination to score embryos across multiple criteria, enabling clinicians to select the most viable embryos for transfer and improve pregnancy outcomes.

When to use it

Embryo grading is applied to all embryos generated in IVF/ICSI cycles at blastocyst stage before transfer. It is essential for prioritizing which embryos to transfer when multiple embryos are available, improving live birth rates per transfer. Grading is particularly valuable when combined with other selection criteria such as preimplantation genetic testing (PGT) or when deciding between fresh and frozen transfer protocols.

What is the difference between a 5AA and a 4BC embryo?

A 5AA embryo is at a more advanced blastocyst stage (5 = fully expanded, versus 4 = early blastocyst), with excellent inner cell mass (A) and excellent trophectoderm (A) quality. A 4BC has lower grades for both ICM and TE quality. The 5AA is generally expected to have higher implantation potential. However, embryo number, patient age, and overall cohort quality also influence transfer decisions.

Can a low-grade embryo still result in pregnancy?

Yes, grade 3 and lower-grade embryos can implant and produce healthy pregnancies, particularly when fewer embryos are available or when dealing with patients with limited ovarian reserve. However, implantation rates decline progressively with lower grades. Clinical context and the alternatives available should guide the decision to transfer a lower-grade embryo.

Why is timing important in embryo grading?

Blastocysts develop rapidly and their morphology changes continuously. A embryo graded as 4BB on the morning of day 5 may progress to 5BB by evening. Delayed imaging or assessment increases the risk of scoring a more advanced stage incorrectly or missing the optimal transfer window. Consistent timing protocols (e.g., morning assessment) improve grading reliability.

Does embryo grading require genetic testing (PGT)?

No, morphological grading and genetic testing (PGT-A) are independent assessments. Many clinics use grading alone, while others combine it with PGT-A for additional information about chromosomal normality. PGT can identify genetically abnormal embryos that appear morphologically normal, providing complementary information for transfer decisions.
