Orthodontic Cephalometry

Cephalometric Analysis for Orthodontics · Also known as: cephalometric analysis, cephalometric radiography, cephalogram

Orthodontic cephalometry is a standardized radiographic technique that produces a lateral or postero-anterior skull radiograph from a fixed source-to-film distance and patient position. Introduced by Benjamin Broadbent in 1931, cephalometric analysis enables systematic measurement of skeletal and dental relationships to assess malocclusion, plan treatment, and monitor growth and treatment changes. The technique remains fundamental to orthodontic diagnosis and treatment planning.

When to use it

Employ cephalometry at the initial diagnostic phase for all orthodontic patients requiring comprehensive treatment planning. Use it to establish baseline skeletal and dental relationships, monitor vertical and sagittal development during treatment, and assess post-treatment stability. Assume adequate radiographic quality without motion blur or positioning errors. Prefer cephalometry when growth prediction or complex three-dimensional changes require systematic documentation.

  • Longitudinal comparisons possible using sequential radiographs to track growth and treatment response

  • Essential for communicating findings to patients and planning interventions
Limitations
  • Two-dimensional representation of complex three-dimensional anatomy introduces anatomical distortion and measurement error
  • Lateral cephalogram cannot reliably assess transverse skeletal relationships or asymmetries
  • Requires standardized head positioning; poor cooperation or anatomical asymmetries compromise reproducibility
  • Prone to tracing errors and observer variability, particularly in identifying subtle landmarks
  • Radiation exposure not insignificant; repeated cephalograms for monitoring increase cumulative dose

What is the difference between lateral and postero-anterior (PA) cephalograms?

Lateral cephalograms assess sagittal (front-back) skeletal relationships and are routine in orthodontics. PA cephalograms view the skull from front to back and assess transverse (width) and vertical asymmetries. Many orthodontists use both for comprehensive assessment.

How reliable are linear and angular cephalometric measurements given measurement error?

Typical measurement error is 1–2 mm for linear measurements and 2–3° for angular measurements. Small differences may reflect measurement variability rather than true anatomical change. Document repeated measurements to assess consistency.

Should cephalometry be repeated for every recall appointment?

Annual or every 18 months is typical for growing patients to monitor growth trajectory. In non-growing patients during active treatment, cephalometry every 1–2 years is reasonable. More frequent radiographs are unnecessary unless specific clinical questions arise.

Can I directly compare cephalometric values from different analysis systems?

No. Different analysis systems define landmarks and reference planes differently, leading to non-equivalent measurements. Choose one system and use it consistently, or calculate normative values specific to your chosen system.
