Occlusal analysis is a systematic clinical and instrumental examination that evaluates the relationships between the maxillary and mandibular teeth, the temporomandibular joint, and the muscles of mastication. Comprehensive occlusal analysis informs diagnosis of malocclusion, temporomandibular disorders, and guides prosthodontic and orthodontic treatment planning. The analysis integrates static occlusal relationships (centric relation, centric occlusion) with dynamic occlusal patterns (jaw movements) to assess functional harmony and identify occlusal interferences.

When to use it
Perform occlusal analysis on all orthodontic and prosthodontic patients, and in patients with temporomandibular disorder symptoms. Repeat analysis after orthodontic treatment, prosthodontic rehabilitation, or TMD therapy to assess outcome. Assume patient cooperation and ability to relax during jaw closure guidance. When severe muscle tension or pain limits centric relation recording, use alternative techniques (e.g., visual estimation or radiographic methods).

What is the difference between centric relation and centric occlusion?
Centric relation is the position where the temporomandibular joint (specifically the condyles) is most stable or physiologically at rest, uninfluenced by tooth contact. Centric occlusion is where the upper and lower teeth meet in maximum intercuspation. In an ideal occlusion, these positions coincide. When they differ, a centric slide occurs during closure.
Is a centric slide problematic?
Small centric slides (< 1 mm) are physiologically normal and generally not problematic. However, larger slides (> 2 mm) or slides with deflection (lateral movement) may create stress on the TMJ and muscles, contributing to TMD symptoms in susceptible individuals. Clinical significance depends on the magnitude, direction, and patient symptoms.
Can occlusal analysis alone diagnose temporomandibular disorder?
No. TMD is multifactorial, involving occlusal factors, muscle tension, stress, postural habits, and joint pathology. Occlusal analysis is one component of comprehensive TMD assessment. A single occlusal finding (e.g., centric slide) does not confirm TMD diagnosis; integrate clinical examination, patient history, and imaging findings.
Should all occlusal interferences be corrected?
Not necessarily. Many occlusal interferences do not cause symptoms or pathology. Correction is indicated when interferences are associated with symptoms (pain, muscle tension, limited opening) or when planning prosthodontic treatment. Conservative approaches (muscle relaxation therapy, behaviour modification) are often recommended before invasive occlusal adjustment.
