| Tóm tắt≠ | The Youth Self-Report (YSR), developed by Thomas Achenbach and Leslie Rescorla, is a youth-completed behavioral rating form assessing emotional and behavioral problems in adolescents aged 11–18 years. Part of the Achenbach System of Empirically Based Assessment (ASEBA), the YSR parallels the parent-completed Child Behavior Checklist (CBCL) and teacher-completed Teacher Report Form (TRF), enabling comprehensive multi-informant assessment of adolescent mental health and functioning. | The Vanderbilt Attention-Deficit/Hyperactivity Disorder (ADHD) Rating Scale, developed by Mark Wolraich and colleagues in 2003, is a validated screening instrument for identifying ADHD symptoms and comorbid behavioral/emotional problems in children aged 6–12 years. Available in parent and teacher versions, it is widely used in primary care, pediatrics, and developmental clinics for initial ADHD screening and symptom tracking during treatment. |
