The VF-14 is a 14-item, disease-specific functional status questionnaire developed to measure visual disability from cataract and its response to cataract surgery. Created by Steinberg, Tielsch, and colleagues (1994), the VF-14 focuses on difficulty with 14 common daily activities (e.g., reading small print, driving day/night, recognizing faces) and is used primarily to assess cataract severity, prioritize surgery, and document post-operative improvement in functional vision.

When to use it
Essential for all cataract patients at baseline to document functional impact and guide surgical decision-making. Repeat at 6–12 week post-operative visit to validate improvement. Use whenever acuity alone does not explain patient complaint (e.g., patient reports severe glare/reading difficulty despite 20/40 acuity—high VF-14 score supports diagnosis of functional cataract). Also applicable to other lens or anterior segment pathology (post-refractive surgery halos, traumatic cataract, secondary cataract). Not intended for posterior segment disease (AMD, diabetic retinopathy), where broader instruments (NEI-VFQ-25) are preferred.
Strengths & limitations
- Pragmatic and activity-specific: items are concrete daily tasks (reading labels, driving, recognizing faces), not abstract concepts—patients and clinicians readily understand scores
- Responsive to cataract surgery: demonstrates substantial improvement (15–40 point changes) post-operatively; used in numerous RCTs as a validated endpoint
- Brief: 14 items, 3–5 minutes administration, feasible in busy ophthalmic practices and surgical centers
- Predictive of satisfaction: high pre-operative VF-14 score combined with symptoms predicts high post-operative satisfaction and low regret rates
- Age-stratified normative data available: reference values for different age groups enable contextualization of individual scores
- Designed for cataract; less valid in other eye conditions (glaucoma, AMD, corneal disease) where functional loss patterns differ
- No subscales; single global score does not distinguish whether disability is visual (acuity/clarity), glare, contrast sensitivity, or peripheral vision loss
- Limited responsiveness to small improvements in mild cataract; floor effect in early disease limits sensitivity
- Does not capture emotional or social QoL impact (e.g., depression, isolation) separate from functional limitation—broader instruments (VFQ-25) needed for comprehensive assessment
- Short-term follow-up data; long-term stability of post-operative improvement beyond 6 months less well-documented

What VF-14 score indicates that cataract surgery is recommended?
No single threshold applies universally. General guidance: VF-14 score >40 combined with patient symptoms (glare, reading difficulty, visual blur) and anatomically appropriate cataract on imaging supports surgery discussion. Scores 25–40 are in a shared decision-making range; patient preferences, other eye status, comorbidities, and life expectancy guide choice. Scores <25 in an asymptomatic patient typically suggest observation.
How much improvement in VF-14 after cataract surgery is expected?
Average improvement is 15–25 points on the 0–100 scale (pre-operative score 50–60 dropping to 25–35 post-operatively). Patients with high pre-operative scores (>60, severe disability) and uncomplicated surgery may improve 30+ points. Some patients with pre-operative scores <30 improve modestly (5–10 points), reflecting baseline mild disease. Individual variation is substantial; complications may blunt improvement.
Can VF-14 be used for eyes with cataract in only one eye (second eye clear)?
VF-14 asks about difficulties without specifying unilateral vs. bilateral. In monocular cataract, VF-14 score reflects functional loss from that eye alone, assuming fellow eye is clear. If fellow eye also has cataract or other pathology, VF-14 reflects combined burden. It is best to note eye status when reporting VF-14 scores.
Is VF-14 available for clinical use, and is there a copyright restriction?
VF-14 is freely available and not proprietary. No licensing or payment required for clinical use or research. Always cite Steinberg et al. (1994) in publications. Multiple-language translations exist with varying validation status.
