Occupational Exposure Questionnaire

Occupational Exposure Questionnaire (OEQ) · Also known as: OEQ

The Occupational Exposure Questionnaire (OEQ) systematically documents workers' exposure to physical, chemical, biological, ergonomic, and psychosocial hazards in their occupational roles. Used by occupational health practitioners and researchers, the OEQ captures frequency, duration, and intensity of hazard exposure, enabling identification of high-risk workers, validation of job exposure matrices, and epidemiological investigation of occupational disease. The OEQ is foundational for occupational health surveillance and regulatory compliance (OSHA, EPA, HSE standards).

When to use it

The OEQ is essential for occupational health surveillance and epidemiological research. Use in: (1) occupational baseline health assessments to establish current hazard exposure baseline and identify workers requiring medical monitoring, (2) regulatory compliance and documentation (OSHA, EPA records), (3) occupational epidemiology studies investigating links between exposures and diseases (e.g., beryllium exposure and chronic beryllium disease, pesticide exposure and cancer), (4) job exposure matrix development for epidemiological research, (5) workplace hazard assessments and validation of engineering or administrative control effectiveness, and (6) occupational health surveillance programs in high-hazard industries (manufacturing, construction, agriculture, healthcare). Particularly critical in workers with long job tenure or cumulative hazard exposure across roles.

Strengths

Limitations
  • Self-report bias: workers may forget or minimize hazard exposure, particularly if they believe reporting will trigger investigation or restrictions. Workers with occupational disease may over-report exposure (recall bias) to explain illness.
  • Temporal and task variation: occupational tasks vary day-to-day; a single questionnaire snapshot may not capture seasonal, cyclical, or event-driven hazard variation
  • Lack of specificity: questionnaire typically asks about 'chemical exposure' or 'heavy lifting' without capturing chemical type or load specifications; validation with objective monitoring is needed
  • Inability to capture past exposures or job histories: OEQ typically measures current exposure; historical exposure (relevant for long-latency diseases like cancer or silicosis) requires job history reconstruction (often poorly recalled)
  • Does not measure actual dose received: frequency and duration estimates are rough proxies for personal exposure dose; without personal air sampling, actual inhaled or dermal dose is unknown (affected by ventilation, PPE use, work practices)

If I report high occupational exposure on an OEQ, will this trigger workplace investigation or restrictions?

The occupational health practitioner's role is to assess hazard exposure and recommend controls, not to punish workers for exposure. High exposure reporting should trigger: (1) review of work practices and engineering controls to determine if exposure can be reduced, (2) medical monitoring recommendation (if exposure is above occupational standards), (3) PPE assessment and training. The goal is to protect your health, not to restrict or blame you. Report honestly; occupational health should respond with support and investigation of root causes (equipment failure, new process, inadequate training), not discipline.

I've been in my job for 20 years with various exposures. How should I report on an OEQ?

Report on your current job conditions. If your questionnaire asks about job history, describe roles and exposures chronologically with estimates of duration (e.g., '2010–2015 as solvent user, daily exposure; 2015–present, occasional exposure due to process change'). Occupational epidemiologists will use job history to reconstruct cumulative exposure. Don't guess exact numbers; approximate ('about 2 hours per day,' 'most days') is sufficient. The occupational health practitioner may ask follow-up questions to clarify duration or intensity; that's normal and helps accuracy.

What's the difference between the OEQ and a workplace hazard assessment?

Hazard assessment identifies what hazards exist in the workplace (e.g., 'we use solvents'); OEQ quantifies how much workers are actually exposed (e.g., 'you use solvents 3 hours daily'). Hazard assessment is a workplace-level review; OEQ is worker-specific. Both are complementary. Hazard assessment guides prevention (engineering controls, PPE); OEQ identifies which workers need medical monitoring based on actual exposure dose.

Can the OEQ be used to establish causation between my exposure and my health condition?

The OEQ alone cannot establish causation, but it is a key component. Establishing occupational disease causation requires: (1) OEQ showing exposure to a hazard known to cause the health condition, (2) dose–response: evidence that higher exposure increases risk, (3) temporality: exposure occurred before disease onset, (4) plausibility: biological mechanism linking exposure to disease, (5) consistency: multiple studies show the association. OEQ provides the exposure component; physicians and epidemiologists interpret all factors together. If you have an occupational health concern, discuss with an occupational medicine physician who can review your exposure, job history, symptoms, and medical evidence holistically.
