Neuropathic Pain Symptom Inventory (NPSI)

Neuropathic Pain Symptom Inventory · Also known as: Neuropathic Pain Symptom Inventory, NPSI Scale

The NPSI is a 12-item self-report questionnaire specifically designed to assess and quantify the diverse symptoms characteristic of neuropathic pain. Developed by Bouhassira and colleagues in 2004, it evaluates five distinct symptom dimensions: burning pain, pressing pain, paroxysmal pain, evoked pain, and paresthesias. The NPSI is widely used in research and clinical practice to characterize neuropathic pain profiles, quantify symptom severity, and track treatment response.

When to use it

NPSI is ideal for detailed characterization of neuropathic pain symptoms in clinical research, particularly in observational studies describing neuropathic pain phenotypes, clinical trials of neuropathic pain treatments to measure treatment efficacy on specific symptom dimensions, and routine clinical care in pain/neurology clinics to guide treatment selection based on symptom profile. It is particularly valuable in conditions with variable neuropathic pain presentations (diabetes, post-herpetic neuralgia, chemotherapy-induced peripheral neuropathy) where matching treatment to symptom profile improves outcomes. Less suitable when only overall pain severity (not symptom specificity) is the outcome of interest; use numeric rating scale for that. Also less suitable as initial screening for neuropathic pain presence (use LANSS or DN4 for diagnostic screening); NPSI quantifies severity in patients already identified as having neuropathic pain.

Strengths & limitations

Strengths
  • Multidimensional structure (five symptom dimensions) enables detailed symptom profiling and reveals heterogeneity of neuropathic pain presentations within and across etiologies
  • Strong psychometric properties demonstrated across diverse neuropathic pain etiologies (diabetic, post-herpetic, chemotherapy-induced, nerve injury) and international populations
  • Directly applicable to treatment selection and monitoring: specific medications target specific symptom dimensions (e.g., anticonvulsants for paroxysmal pain)
  • Brief administration (5-10 minutes) with straightforward 0-10 numeric rating scale makes it practical for clinic and research use
  • Extensively translated (25+ languages) and used internationally, enabling cross-cultural neuropathic pain research
Limitations
  • Designed for patients with confirmed neuropathic pain; less suitable as initial screening instrument for neuropathic vs. nociceptive pain differentiation
  • Does not assess functional impact or quality-of-life consequences of neuropathic pain; supplements with disability measures for comprehensive assessment
  • Five-dimensional structure requires interpretation of multiple scores; some clinicians prefer single summary measures for simplicity
  • Correlation between total NPSI and treatment efficacy varies by medication and symptom dimension; improvement in one domain does not guarantee improvement in others

Frequently asked

If a patient's NPSI total score improves but individual dimensions remain unchanged, what does that mean?

This is unlikely but possible due to scoring artifacts or measurement error. More likely, if total NPSI changes, at least one dimensional score changed meaningfully. However, if dimensional scores remain stable despite patient reporting subjective improvement, the patient may be experiencing functional or quality-of-life gains not captured by symptom severity alone. Always ask the patient directly about improvement; NPSI is one measure among many.

How do I choose which medication based on NPSI subscore pattern?

General principles: High Paroxysmal subscore (shooting, stabbing) → consider anticonvulsants (gabapentin, pregabalin). High Burning subscore → consider tricyclic antidepressants or SSRIs. High Evoked pain (pain to touch) → consider topical treatments (lidocaine patch). High Paresthesias → various agents may help. Many patients have mixed profiles and require combination therapy or sequential trials of different medication classes.

Can NPSI detect if neuropathic pain is improving with treatment?

Yes. Serial NPSI assessment (baseline, weeks 4-8 during treatment, then periodically) tracks whether specific symptom dimensions improve. Improvement in one or more dimensions indicates treatment is working on that component. Lack of improvement after adequate trial duration suggests inadequate efficacy or need for medication adjustment or alternative. NPSI is sensitive to treatment-related symptom change.

What is the relationship between NPSI and functional disability or quality of life in neuropathic pain?

NPSI quantifies symptom severity; disability and QoL are separate outcomes. A patient with low NPSI (mild neuropathic pain symptoms) may have high disability if symptoms are in functionally critical areas (feet, hands). A patient with moderate NPSI may have low disability if symptoms are minor. For comprehensive neuropathic pain assessment, measure symptoms (NPSI), function (pain interference), and quality of life (PDQ, SF-36) together.
