Available in parent and teacher versions, it is widely used in primary care, pediatrics, and developmental clinics for initial ADHD screening and symptom tracking during treatment. | The Youth Self-Report (YSR), developed by Thomas Achenbach and Leslie Rescorla, is a youth-completed behavioral rating form assessing emotional and behavioral problems in adolescents aged 11–18 years.
