Patients rate their pain on a scale from 0 to 10, where 0 represents no pain and 10 represents the worst pain imaginable. The NRS is one of the most frequently used pain assessment instruments in clinical practice due to its brevity, ease of administration, and strong psychometric properties across diverse patient populations.The Bates-Jensen Wound Assessment Tool (BJWAT), originally developed as the Pressure Sore Status Tool, is a comprehensive instrument for objectively assessing pressure ulcer characteristics and monitoring healing progress. Created by Barbara M. Bates-Jensen, the tool evaluates 13 distinct wound dimensions including size, depth, tissue type, and exudate. It provides quantitative assessment of wound severity and is useful for tracking changes over time and evaluating treatment effectiveness.
