Home Food Environment Questionnaire

Also known as: HFEQ, Home Food Environment, Food Environment Assessment

The HFEQ is a parent-report questionnaire measuring the household food environment—the availability of healthy and unhealthy foods, parent feeding practices, and family mealtime characteristics. Developed by Boles, Fulkerson, and colleagues, the HFEQ captures multiple dimensions of the home environment that influence children's dietary intake and weight status. The home food environment is a key modifiable target for childhood obesity prevention interventions.

When to use it

The HFEQ is appropriate for research and intervention targeting childhood obesity prevention through environmental modification. Use in cross-sectional studies examining associations between home food environment and child weight status or dietary intake. Suitable for baseline and post-intervention assessment in obesity prevention programs; measure whether parenting classes or nutrition interventions successfully improve the home food environment. Appropriate for parent-focused interventions (parenting skills training, nutrition education, food assistance) to quantify environmental change. Can be used in clinical pediatric nutrition assessment to understand barriers to healthy eating at home.

  • Does not measure actual food intake or child weight; environment is proximal cause but not deterministic.
  • Does not assess food affordability or access barriers; high scores for 'healthy food availability' may not reflect feasibility for low-income families.
  • Does not measure quality of parent-child mealtime interaction beyond broad feeding practice items; cultural and emotional aspects are limited.
  • Assumes parents are primary food gatekeeper; less applicable in childcare settings or for children with high autonomy.
  • Does not distinguish between purchased foods and home-prepared meals; some items may conflate availability with use.

Frequently asked

How do I interpret a parent's report of high healthy food availability if they also report very limited food budget or access?

High reported availability may reflect parents' intentions or aspirational reporting (social desirability bias) rather than actual sustained availability. Ask follow-up questions about frequency (Are these foods in your home every week? Once a month?), storage limitations (Do you have adequate refrigerator space?), and affordability (Can your budget sustain regular purchases of fresh produce?). Combine HFEQ with questions about barriers (food cost, access to stores) to triangulate actual environmental constraints.

Is the restriction subscale always bad, or is some parental restriction of unhealthy foods appropriate?

Moderate restriction (limiting sugary drinks, fried foods, desserts) is developmentally appropriate and associated with healthier eating. Excessive restriction (completely forbidding foods, creating secrecy around eating) can backfire, associated with overconsumption when restrictions lift or disordered eating patterns. HFEQ subscales measure the construct but do not inherently judge; interpret restrictive feeding within context of child development and parent-child relationships. Very high restriction scores warrant discussion about balanced, flexible approaches.

Can HFEQ be used for children of different ages, or does it need age-specific modification?

Standard HFEQ is typically validated for school-age children (6–12 years). For preschoolers (<5 years), some items may not apply (e.g., 'I monitor what snacks my child eats' is less relevant if parent controls all food access). For adolescents (13+), autonomous food choices (peers, school, allowance) become more salient; HFEQ may miss these influences. Consider age-specific validation or modifications; use published versions tested in your target age group.

How does the home food environment relate to school lunch intake or food served outside the home?

HFEQ measures home environment specifically; it does not capture school, restaurant, or peer-shared foods. Home environment is one modifiable domain but is not the only influence on child diet. For comprehensive assessment of dietary intake and weight-related behaviors, pair HFEQ with dietary intake data (24-hour recall) and information about school meal access, amount of screen time, and physical activity.
