Timed Up and Go Test

Timed Up and Go (TUG) Test · Also known as: TUG, TUG test

The Timed Up and Go (TUG) test is a simple, quick performance assessment that measures the time required to stand from a chair, walk 3 meters, turn around, and return to sitting. Developed by Podsiadlo and Richardson in 1991, the TUG has become one of the most widely used tests in geriatric and rehabilitation settings for assessing mobility, balance, and fall risk in older adults and individuals with mobility limitations.

When to use it

The TUG is indicated for older adults (age 60+), individuals with gait disturbances, balance disorders, or post-stroke rehabilitation. It is less useful for patients unable to ambulate independently or those with severe cognitive impairment who cannot follow instructions. The test is appropriate for community, outpatient, and inpatient settings.

Strengths
  • Extremely simple to administer with minimal space and no special equipment required
  • Takes only 3-5 minutes including instructions and practice; efficient for busy clinics
  • Predicts fall risk and identifies elderly individuals at high risk for adverse outcomes
  • Directly assesses integrated functional mobility relevant to community participation
Limitations
  • Ceiling effects for highly functional individuals; limited discrimination at the upper end
  • Does not provide specific information about underlying impairments (strength vs. balance vs. speed)
  • Requires safe standing balance and ability to follow commands; not suitable for severe impairment
  • Single time point subject to day-to-day variation; multiple trials or conditions may be needed for reliable assessment

Can the patient use an assistive device like a walker during the TUG?

Yes, patients should use whatever assistive device they normally use for walking. However, the device must be documented because it affects interpretation; TUG times are substantially different for walkers versus canes versus no aid. Comparisons between timepoints require consistent device use.

What does a TUG time of 20 seconds mean?

A TUG time of 20 seconds is in the gray zone between low and high fall risk (boundary is typically 12-14 seconds for very fit elderly, 21 seconds for older community-dwellers). This person likely has some mobility impairment and would benefit from intervention. The exact interpretation depends on age, comorbidities, and baseline function.

Should the patient practice before the timed trial?

Yes, one or two practice trials familiarize the patient with the task and reduce learning effects. The timed trial should be performed after the patient demonstrates understanding of the task. Some patients show faster times on repeat testing due to familiarity.

How often should TUG be repeated during rehabilitation?

Reassessment frequency depends on treatment intensity and expected progression. For inpatient rehabilitation with daily therapy, weekly testing is reasonable. For outpatient settings, monthly or every 4-6 weeks allows detection of meaningful change. Testing too frequently may be affected by fatigue or day-to-day variation.
