Patient Fall Risk Assessment

Comprehensive Patient Fall Risk Assessment Protocol · Also known as: Fall Risk Screening, Fall Prevention Assessment, PFRA

Patient Fall Risk Assessment is a systematic clinical evaluation process used to identify hospitalized or institutionalized patients at increased risk of falling. Falls are a major cause of injury and mortality in healthcare settings, particularly among older adults. The assessment considers intrinsic patient factors (e.g., age, medical conditions, medications) and extrinsic environmental factors (e.g., lighting, equipment, flooring) to guide preventive interventions.

When to use it

Conduct fall risk assessment upon hospital admission, periodically during stay (especially after changes in status), and at discharge for high-risk patients. Apply universally in acute care, long-term care, and rehabilitation settings. Reassess following any adverse event, change in medication, or mobility status.

How often should fall risk assessment be repeated during hospitalization?

Can fall risk assessment predict all falls?
