The Malnutrition Universal Screening Tool (MUST), developed by Elia and endorsed by BAPEN (British Association for Parenteral and Enteral Nutrition), is a rapid 3-component screening tool for identifying adults at risk of malnutrition in hospital and community settings. When to use it
MUST is used in hospital inpatient and outpatient settings, as well as community and primary care, to identify all adults at nutritional risk. It is particularly valuable for rapid screening in busy environments because of its simplicity and reproducibility. MUST can be performed by any healthcare worker or even by patients themselves (with guidance). Re-assessment is recommended during prolonged hospitalization or community care.

- Depends on accurate current and usual weight; patients may not know usual weight or weight loss may not be documented
- Acute illness component is subjective; definition of 'reduced intake' can vary between assessors
- Less detailed than comprehensive nutritional assessment; MUST identifies risk but not specific nutrient deficiencies

My MUST is low but I feel weak; do I need nutrition support?
MUST identifies risk of malnutrition based on weight loss and BMI, not all causes of weakness. Weakness may be from other causes (anemia, muscle weakness, medication). Discuss your symptoms with your doctor; MUST is one tool, not the full picture.
How do I calculate weight loss if I don't know my usual weight?
Ask the patient if possible, or check medical records for recent weights. If unavailable, use an estimate or medical judgment. Document that usual weight was estimated. Some clinicians use current weight compared to weight 3 months ago from prior visits.
What's the difference between MUST and NRS-2002?
Both are valid. MUST is UK/European standard, emphasizing weight loss and BMI. NRS-2002 is European standard, emphasizing disease severity and age. MUST is preferred in UK/community settings; NRS-2002 is preferred in hospitals and EU. Both identify nutritional risk.
Can MUST be used for pediatric patients?
No. MUST is designed for adults only. Pediatric malnutrition screening uses different tools (e.g., STRONGkids) because growth and metabolism differ.
