Created by Barbara M. Bates-Jensen, the tool evaluates 13 distinct wound dimensions including size, depth, tissue type, and exudate. It provides quantitative assessment of wound severity and is useful for tracking changes over time and evaluating treatment effectiveness. | The Braden Scale is a standardized risk assessment instrument used in nursing to identify hospitalized patients at risk of developing pressure ulcers.
