Weighed Food Record

The Weighed Food Record (WFR) is a prospective dietary assessment method in which the respondent weighs and records every food and beverage at the moment of consumption, weighing back any leftovers, to obtain a precise quantitative measure of intake. Because it removes the portion-size guesswork that limits recalls and food-frequency questionnaires, it is widely regarded as the reference or gold-standard method against which other dietary instruments are validated. Sheila Bingham and colleagues' classic studies in the British Journal of Nutrition compared weighed records with recalls and questionnaires and, using the 24-hour urinary nitrogen biomarker, demonstrated that weighed records most closely track an objective measure of protein intake. The cost is high respondent burden and the risk that the act of weighing changes what people eat.

When to use it
Use a weighed food record when you need the most accurate practical measure of an individual's intake and can accept high respondent burden — for example, as the criterion method to validate or calibrate food-frequency questionnaires and recalls, in tightly controlled metabolic or clinical studies, or where precise absolute intakes are essential. It is appropriate with literate, motivated, well-trained participants who can be given scales and detailed instructions, ideally over several non-consecutive days to capture variation. It is poorly suited to large population surveillance, low-literacy or low-resource settings, and any context where the weighing burden would reduce participation or alter eating behavior. When a rapid, low-cost diet-quality proxy is enough, a dietary diversity score is preferable; when burden must be lower but quantitation is still needed, a multiple-pass recall is the usual choice.

Why is the weighed food record considered a gold standard?
Because it replaces estimation with measurement. Respondents weigh each food before eating and weigh back leftovers, so the consumed amount is known to the gram rather than guessed, removing the largest source of error in recalls and food-frequency questionnaires. Bingham and colleagues showed that weighed records agree more closely than other self-report methods with the 24-hour urinary nitrogen biomarker, an objective external criterion for protein intake. This combination of direct measurement and biomarker validation is why it is used as the reference against which cheaper instruments are judged.
What is reactivity and why does it matter here?
Reactivity is the tendency of people to change their eating because they are recording it. Having to weigh and write down every item is burdensome and conspicuous, so respondents may eat simpler meals, smaller portions, or fewer snacks during the recording period than they normally would. This means even a perfectly executed weighed record can underestimate or distort usual intake. It is a fundamental limitation of any prospective self-recorded method and a reason weighed records, despite their precision, are not automatically a perfect measure of habitual diet.
How does it differ from an estimated diet record?
Both are prospective records kept as foods are consumed, but they differ in how amounts are captured. A weighed record uses a scale to measure the exact grams served and left over, while an estimated record describes portions in household measures such as cups, spoons, or slices. The estimated record is far less burdensome and more feasible at scale, but it reintroduces portion-size estimation error. Bingham's comparison placed the weighed record ahead of the estimated record in agreement with biomarkers, reflecting the value of actual weighing.
