Wheelchair Skills Test

The Wheelchair Skills Test (WST) is a standardized, objective assessment of how well a wheelchair user can perform a graded set of individual wheelchair skills, from basic maneuvers like rolling forward and turning to advanced ones like descending curbs and performing a stationary wheelie. Developed by R. Lee Kirby and colleagues at Dalhousie University and validated through a 2002 pilot and a 2004 measurement-properties study of version 2.4, the WST scores each skill for whether the user can perform it and how safely, then summarizes performance as a percentage of skills passed. It turns the diffuse notion of wheelchair mobility into a reliable, comparable, and trainable outcome.

Use the WST when you need an objective, standardized measure of a wheelchair user's skill capacity — for baseline assessment, evaluating training programs, comparing devices or configurations, or research on wheelchair mobility. It applies to manual and powered wheelchair users, with corresponding skill sets, and the questionnaire form (WST-Q) is useful when a physical test course is impractical or when you want self-reported performance and confidence. It is less suited when the question is satisfaction or psychosocial impact (use QUEST or PIADS), when no safe testing environment is available for the performance version, or when the user's relevant skills fall entirely outside the standardized set.

What does the WST actually measure?
It measures a wheelchair user's capacity to perform a standardized, graded set of individual wheelchair skills — from rolling and turning to negotiating curbs and performing a wheelie — and how safely they do so. Each skill is scored pass, fail, or not applicable, with a safety rating, and the results are summarized as the percentage of applicable skills passed. This converts the broad idea of wheelchair mobility into a reliable, comparable number that can be tracked and targeted with training, rather than a subjective impression of how well someone gets around.
What is the difference between the WST and the WST-Q?
The WST is a performance test in which an assessor observes the user attempting each skill on a standardized course; the WST-Q is a questionnaire version in which the user or a caregiver reports on skills without a physical test. The WST-Q can capture not only capacity but also performance (whether the user actually does the skill in daily life) and confidence. The questionnaire form is valuable when a safe test course is impractical, while the performance test gives objective, observed capacity. They measure overlapping but distinct constructs.
Why record safety separately from whether a skill is passed?
Because a user can complete a skill in a way that is risky — for instance descending a curb but nearly tipping. A mobility measure that only recorded completion would treat that as equivalent to a controlled, safe descent, hiding important risk information. The WST scores capacity and safety separately so that an unsafe pass is flagged. This matters clinically, since a key goal of wheelchair-skills assessment and the companion training program is not just expanding the skill repertoire but ensuring skills are performed safely.
