Direct standardization is a demographic technique that makes summary rates comparable across populations by applying each population's group-specific rates — most often age-specific death or disease rates — to a single, common standard population structure. The resulting directly standardized rate answers a counterfactual question: what would the crude rate be if every population had the same age (or other) composition? It removes the confounding effect of differing population structure so that genuine differences in underlying risk can be compared on a level footing.

When to use it

Use direct standardization when comparing summary rates (mortality, incidence, fertility) across populations or over time and the populations differ in age or other compositional structure, and when you have reasonably stable group-specific rates for each population. It is the preferred standardization method whenever the study population's own group-specific rates are reliable. Assumptions: group-specific rates are estimated with acceptable precision, group boundaries are comparable across populations, and a meaningful common standard exists. Do NOT use direct standardization when the study population's group-specific rates are based on very small counts (use indirect standardization instead), and never interpret the standardized rate as a real rate for any actual population — it is a hypothetical comparison index whose absolute value depends entirely on the arbitrary standard chosen.

What is the difference between direct and indirect standardization?

Direct standardization applies the study population's own group-specific rates to a standard population structure, so it requires reliable group-specific rates. Indirect standardization does the reverse: it applies a standard set of group-specific rates to the study population's own structure and yields a standardized mortality ratio (SMR). The indirect method is preferred when the study population's group-specific counts are too small to give stable rates.

Does it matter which standard population I choose?

It matters for the absolute value of the standardized rate but not for the qualitative comparison, provided the same standard is used for all populations. Different standards (e.g., WHO World Standard versus a European Standard) put more or less weight on older ages and so produce different numerical rates. The cardinal rule is to use one standard consistently and to state which standard was used.

Can the difference between two standardized rates be decomposed further?

Yes. Direct standardization removes composition so that the standardized-rate difference reflects the rate (risk) component, which is precisely the rate effect isolated by Kitagawa decomposition. Kitagawa's method makes the connection explicit by also quantifying the composition component, so standardization can be seen as reporting one part of a full decomposition.
