ResuméMedication Reconciliation is a systematic process of identifying and resolving discrepancies between the medications a patient should be taking and what they are actually taking. Endorsed by The Joint Commission as a National Patient Safety Goal, medication reconciliation occurs at critical transition points such as hospital admission, transfer between units, and discharge. The process reduces medication errors and adverse drug events that can result from omissions, duplications, or interactions.Nursing-Sensitive Indicators are quality metrics that measure healthcare outcomes significantly influenced by nursing care. Developed by the American Nurses Association (ANA) and maintained through the National Database of Nursing Quality Indicators (NDNQI), these indicators assess hospital-acquired complications, staffing levels, nurse-sensitive outcomes, and other dimensions of care quality. They serve as benchmarking tools for evaluating nursing practice effectiveness and organizational performance.
