Wells Score for DVT
Also known as: Wells DVT Score, DVT Wells
The Wells score, developed by Wells et al. in 1994, is a clinical prediction rule that stratifies patients into low, intermediate, or high pretest probability of deep vein thrombosis (DVT). It combines seven clinical features to guide diagnostic testing decisions and reduce unnecessary imaging in suspected DVT patients.

When to use it
The Wells score is used in emergency departments, primary care, and urgent care settings to guide evaluation of unilateral leg swelling. It is most useful when coupled with D-dimer testing: low Wells score + negative D-dimer = no imaging needed. However, if imaging facilities are not available or if clinical suspicion is very high despite Wells score, imaging should not be withheld.

- Does not account for unilateral bilateral lower extremity swelling or mass effects from other pathology

If Wells score is high, do I still need D-dimer?
If Wells score is high (≥ 3), D-dimer test is often skipped and patients proceed directly to ultrasound because high pretest probability makes D-dimer less helpful. However, if ultrasound is not immediately available, a D-dimer can sometimes help triage urgency. Always follow your institutional protocol.
Can I exclude DVT with Wells score alone (without D-dimer)?
No. Wells score alone is not sufficient to exclude DVT. The combination of low Wells score AND negative D-dimer is what safely excludes DVT. A low Wells score with unavailable or positive D-dimer still warrants ultrasound.
What if the patient has bilateral leg swelling?
Wells score is designed for unilateral swelling. Bilateral leg swelling suggests alternative diagnoses (heart failure, lymphedema, venous obstruction) or bilateral DVT. Assess for alternative causes and obtain imaging if DVT is still suspected.
Is Wells score accurate for cancer patients?
Wells score includes 'active cancer' as a point; cancer patients are inherently higher risk. However, D-dimer is less specific in cancer patients (often elevated without DVT). A moderate or high Wells score warrants ultrasound regardless of D-dimer in cancer patients.
