CHA₂DS₂-VASc Score
Also known as: CHA2DS2VASc, Atrial fibrillation stroke risk
The CHA₂DS₂-VASc score, developed by Lip, Nieuwlaat, and colleagues in 2010, is a 9-point risk stratification tool for predicting annual stroke and systemic thromboembolism risk in patients with atrial fibrillation. It is the recommended score by major cardiology guidelines for guiding anticoagulation decisions.

When to use it
CHA₂DS₂-VASc is applied to all patients with atrial fibrillation (paroxysmal, persistent, or permanent) to guide anticoagulation decisions. It is equally applicable in emergency departments for new-onset atrial fibrillation, outpatient cardiology clinics, and primary care. CHA₂DS₂-VASc is less applicable to patients with contraindications to anticoagulation, for whom the score informs risk-benefit discussion.

My CHA₂DS₂-VASc is 1; do I need anticoagulation?
CHA₂DS₂-VASc = 1 in males is considered low risk and anticoagulation is generally not recommended. However, if CHA₂DS₂-VASc = 1 due to a female patient or age 65-74, reassess more carefully. Some clinicians offer aspirin as a lower-risk alternative, though anticoagulation is preferred if tolerated.
Does my CHA₂DS₂-VASc score change if I'm anticoagulated?
No. CHA₂DS₂-VASc score itself does not change with anticoagulation; it is a static risk assessment at a point in time. However, anticoagulation is the result of the CHA₂DS₂-VASc assessment and effectively reduces stroke risk by ~60–80%.
What is HAS-BLED and how does it relate to CHA₂DS₂-VASc?
HAS-BLED assesses bleeding risk during anticoagulation (hypertension, abnormal renal/liver function, stroke, bleeding history, labile INR, elderly age, drugs/alcohol use). CHA₂DS₂-VASc assesses stroke risk; both should be evaluated to make balanced anticoagulation decisions.
Should I anticoagulate a patient with AF and CHA₂DS₂-VASc ≥ 2 even if they have contraindications?
High CHA₂DS₂-VASc is strong indication for anticoagulation, but absolute contraindications (e.g., active bleeding, severe thrombocytopenia) must be respected. Work to resolve contraindications or consider bridging strategies (e.g., temporary heparin with planned DOAC when contraindication resolves).
