Mechanisms
Gingivitis develops when microbial plaque accumulates undisturbed along the gingival margin, triggering a local host inflammatory response with vasodilation, increased crevicular fluid, and infiltration of inflammatory cells. The classic experimental gingivitis study (Löe et al., 1965) demonstrated the cause-and-effect link: when participants stopped oral hygiene, plaque accumulated and gingivitis developed within days, and reinstating plaque removal reversed it.
