EORTC QLQ-CX24: Quality of Life in Cervical Cancer

The EORTC QLQ-CX24 is a 24-item cervical-cancer-specific module designed to complement the EORTC QLQ-C30 core questionnaire. Developed by Greimel et al. in 2006, it measures sexual/vaginal function, body image, lymphedema, neuropathy, and gastrointestinal symptoms specific to cervical cancer and its treatments. It is the standard cervical cancer-specific QoL instrument in international oncology research.

When to use it
EORTC QLQ-CX24 is indicated for women with cervical cancer (any stage) undergoing surgery (hysterectomy, trachelectomy), pelvic radiation, chemotherapy, or surveillance. Primary use is clinical trials assessing treatment efficacy and patient-reported outcomes, particularly in gynecologic oncology research; secondary use in oncology clinic monitoring and supportive care research. NOT for screening or diagnosis. Particularly valuable when treatment decisions involve sexual/reproductive function trade-offs (e.g., radical hysterectomy vs. chemotherapy/radiation; fertility-sparing surgery options; adjuvant radiation impact).

Yes. QLQ-CX24 is validated across cervical cancer age groups, including young women (20s-40s). Sexual/Vaginal Functioning scale captures reproductive/sexual impact. However, QLQ-CX24 does not specifically assess fertility outcomes or reproductive planning. Supplement with fertility-specific questions or oncofertility counseling for comprehensive reproductive health assessment.
How does QLQ-CX24 compare to other gynecologic cancer QoL instruments?
EORTC QLQ-CX24 is cervical-cancer-specific. EORTC QLQ-OV28 (ovarian cancer) and QLQ-EN24 (endometrial cancer) exist for other gynecologic malignancies, each with disease-specific item focus. QLQ-CX24 is the standard for cervical cancer; do not use for other gynecologic cancers without validation.
What is the MCID for QLQ-CX24 scales?
MCID for QLQ-CX24 scales is approximately 10–15 points on 0–100 range. However, MCID may vary by scale; sexual/vaginal function may have different meaningful thresholds than symptom scales. In trials, MCID helps determine clinical significance of treatment-related changes.
Can QLQ-CX24 be used in patients undergoing fertility-sparing surgery (trachelectomy)?
Yes. QLQ-CX24 is appropriate for all cervical cancer surgical approaches, including fertility-sparing trachelectomy. Sexual/vaginal functioning scores may differ between hysterectomy and trachelectomy cohorts; ensure adequate statistical power and stratification if comparing approaches.
How are missing data handled in QLQ-CX24 scoring?
EORTC scoring guidelines: if ≥50% of items in a scale are missing, do not score that scale. If <50% are missing, impute the mean of completed items. Sexual/Vaginal Functioning scale (6 items) should have low missing rates; if significant missing data, consider whether respondents declined due to discomfort and use sensitivity analysis.
