The underlying (input) form is the abstract representation stored in memory and what you intend to say (e.g., /impaʃ/ 'impasse'). The surface (output) form is what actually comes out—the observed pronunciation, subject to constraints (e.g., [empaʃ] or [impaʃ] depending on the language). OT selects the optimal output from candidates generated from the input.

How do I determine the correct constraint ranking?

Examine minimal pairs and alternations where two constraints conflict. The candidate that appears in the language must satisfy the higher-ranked constraint, even if it violates the lower-ranked one.
