DALY Computation

The disability-adjusted life year, or DALY, is the central metric of the global burden of disease, expressing the total health loss from a disease, injury, or risk factor as a single time-based number. One DALY is one healthy year of life lost. The metric, developed by Christopher Murray and elaborated with Alan Lopez in the 1990s, combines two distinct kinds of loss on a common scale: the years of life lost when people die earlier than a reference life expectancy, and the years of healthy life lost when people live with illness or disability rather than in full health. By weighting time lived in poor health by its severity and adding it to time lost to premature death, the DALY makes it possible to compare conditions as different as a fatal cancer and a chronic non-fatal disease, and to rank causes of ill health across populations in a way mortality statistics alone cannot.

When to use it

Use DALY computation when you need a single, comparable measure of population health loss that captures both premature death and time lived in poor health, for example to set health priorities, allocate resources, evaluate interventions through cost-per-DALY-averted analyses, or track the burden of diseases and risk factors over time. It is the appropriate metric when fatal and non-fatal conditions must be ranked on a common scale and when comparisons across causes, regions, or years are the goal. DALY accounting depends on having credible inputs, cause-specific mortality, incidence or prevalence, durations, and disability weights, so it is best suited to settings with reasonable surveillance or modeled estimates. It is less appropriate when those inputs are too weak to support credible weights and durations, when the value judgments embedded in standard life expectancies and disability weights are unacceptable to stakeholders, or when an individual-preference-based measure for clinical decision-making is wanted, where the quality-adjusted life year is the more natural choice.

What is the difference between YLL and YLD?

Years of life lost (YLL) measure the mortality burden: for each death, the years between the age at death and a standard reference life expectancy, summed over deaths. Years lived with disability (YLD) measure the non-fatal burden: the time people spend living with a condition, weighted by its severity through a disability weight, and computed from incidence times duration or from prevalence. YLL captures dying too soon; YLD captures living in less than full health. A DALY is simply their sum, putting both forms of health loss in the same unit of healthy years lost.

What is a disability weight and where does it come from?

A disability weight is a number between zero and one that represents the severity of a health state, where zero is full health and one is a state regarded as equivalent to death. A weight of 0.3, for instance, means a year lived in that state counts as losing thirty percent of a healthy year. Murray established the technical basis for these weights; early versions came from expert valuation exercises, while later Global Burden of Disease rounds derived them from large general-population surveys asking people to compare health states. They are the device that converts time lived in illness into comparable lost healthy time.

Why did later studies drop age weighting and discounting?

The original DALY gave more value to years lived in young adulthood (age weighting) and valued near-term health gains above distant ones (discounting), both explicit social-value choices Murray built into the formula. They were always controversial because they imply, for example, that saving a young adult counts for more than saving an infant or an elderly person. To make the metric more transparent and equitable and to avoid embedding contested value judgments, later Global Burden of Disease studies set both the age-weighting modulation and the discount rate to zero, so a year of healthy life now counts equally regardless of age or timing, though analysts may reintroduce them in sensitivity analyses.
