A retrospective case series is an observational study that systematically describes the clinical features, treatments, and outcomes of a defined group of patients by examining pre-existing medical records or administrative data. It looks backward in time — data have already been recorded before the study begins. With no control group, no randomization, and no prospective follow-up, it sits near the base of the evidence hierarchy but remains one of the most practical and frequently published study designs in clinical medicine.

When to use it

Use a retrospective case series when you need to describe the natural history, clinical spectrum, or outcomes of a specific condition or intervention and only existing records are available. It is appropriate for rare diseases or rare complications where prospective enrollment is impractical, for hypothesis generation ahead of a controlled study, and for initial safety/feasibility reporting of novel procedures. Do not use it to test causal hypotheses, to estimate incidence or prevalence in a population, or when a comparison group is scientifically required — those questions demand cohort or case-control designs. Avoid this design when record completeness is poor or when outcome ascertainment is likely to be systematically biased by the retrospective mode of data capture.

What is the difference between a retrospective case series and a retrospective cohort study?

A retrospective cohort study defines two or more groups based on past exposure status and compares outcomes between them, enabling estimation of relative risk. A retrospective case series describes one group of patients with a shared condition or procedure without any comparison group. Cohort studies can support causal inference (albeit with limitations); case series cannot.

How many cases are needed for a case series?

There is no minimum enforced by study design — even three or four cases have been published. In practice, larger series (20 or more cases) carry more descriptive value and are more credible. The number should be large enough to capture the range of presentations and outcomes relevant to the clinical question, not chosen to reach a target p-value.

Can I calculate a p-value or confidence interval in a case series?

Descriptive statistics are appropriate: frequencies, proportions, medians, ranges, and confidence intervals around proportions. Hypothesis tests comparing groups are not appropriate because there is no comparison group. Confidence intervals can quantify uncertainty around single proportions (e.g., a complication rate), but should not be misread as evidence of a treatment effect.

What reporting guideline should I follow?

The PROCESS (Preferred Reporting Of CasE Series in Surgery) checklist, published by Agha et al. in 2016 in the International Journal of Surgery, is the most widely endorsed guideline for surgical case series. More general clinical case series may also benefit from CARE (CAse REport) guidance or journal-specific instructions.

When should I upgrade from a retrospective case series to a controlled design?

As soon as the clinical question shifts from 'What happened?' to 'Does this treatment work better than another?' or 'What factors predict outcome?', a controlled design is required. Upgrade to a retrospective cohort or case-control study when a comparison group can be identified from existing records, and to a prospective design when you need complete, research-quality data collection.
