The ICF linking rules are a standardized procedure, introduced by Alarcos Cieza and colleagues at the ICF Research Branch and refined over subsequent years, for mapping the content of health and functioning instruments onto the categories of the International Classification of Functioning, Disability and Health. By extracting the meaningful concepts contained in each item and assigning them to the most precise matching ICF category, the rules make it possible to compare what different questionnaires actually measure, to express scores in a common functioning language, and to detect content that the ICF does not cover, such as personal factors. They are the bridge between the world's many disability instruments and the single ICF reference framework.

When to use it

Use the ICF linking rules when you need to know what an instrument actually measures in ICF terms, to compare the content of two or more measures, to harmonize legacy datasets onto a common functioning metric, or to evaluate whether a questionnaire covers the domains relevant to a condition (often against an ICF Core Set). They are appropriate for any text that expresses functioning concepts — items, response options, goals, clinical notes. They are not a scoring method and do not by themselves equate scores; they establish content correspondence. Their reliability depends on trained coders, and concepts outside the ICF (notably personal factors) can only be flagged, not linked.

Do the linking rules make instrument scores comparable?

No. The rules establish what concepts an instrument measures in ICF terms, which is content correspondence, not metric equivalence. Two instruments may link to the same ICF categories yet use different response scales and scoring, so making their scores comparable requires additional psychometric work such as common-item equating or item-response-theory calibration. Linking is the necessary first step that tells you whether such equating is even meaningful.

What happens to concepts the ICF does not classify?

They are explicitly labelled. The refined rules provide categories for concepts that are not covered by the ICF, not sufficiently defined, that are personal factors (which the ICF deliberately leaves unclassified), or that denote a health condition. Flagging rather than force-fitting these concepts is one of the method's strengths: it prevents misleading links and documents exactly where instruments measure things beyond the ICF's scope, such as personal factors.

Why is inter-coder agreement important here?

Because linking is interpretive, a single coder's choices could reflect idiosyncratic judgment. Having two or more trained coders link independently and reporting their agreement (commonly with kappa) demonstrates that the mapping is reproducible. Disagreements are resolved by discussion or a third coder. Without this step a content comparison cannot be trusted, so agreement assessment is built into the standard procedure.
